BC 8 - Application for Prequalification

Firms wishing to bid on Illinois Department of Transportation highway construction contracts must prequalify by submitting a properly completed BC 8 (Contractor’s Statement of Experience and Financial Condition).

  • The properly completed BC 8 must be received in the Bureau of Construction no later than 4:30 PM 21 days prior to the date of the letting in question.
  • The documents must be properly signed and notarized. Faxes and emails are not allowable.

BC 2267 - Certificate of Joint Venture

Joint venture of prequalified firms wishing to be authorized to bid on Illinois Department of Transportation highway construction contracts must submit properly completed copies of the Form BC 2267 (Certificate of Joint Venture).

  • A separate form must be submitted on each letting item for which authorization to bid is sought.
  • A properly signed and notarized original of each form must be received by the department no later than 4:30 p.m. prevailing time at least seven (7) days prior to the date of the letting of interest.
  • Further, each party to the joint venture must submit a properly completed, signed and notarized original copy of the BC 57 (Affidavit of Availability) by the aforementioned deadline.

BC 57 - Affidavit of Availability

Prequalified firms wishing to be authorized to bid on Illinois Department of Transportation highway construction contracts must submit properly completed copies of the BC 57 (Affidavit of Availability).

  • Firms must list all currently uncompleted work and currently pending awards on the form.
  • A properly signed and notarized original copy must be received, along with a properly completed copy of the BD 124 (Request for Proposal Forms and Plans & Request for Authorization to Bid) form, by the department no later than 4:30 p.m. prevailing time three (3) days prior to the date of the letting in question.
