BMPR MT01: Fastener Identification Form

Body:
The purpose of this form is to identify fastener samples
that are to be tested by the Bureau of Materials & Physical
Research for contact conformance that are being supplied
from out of state sources. The Fastener Identification Form
is primarily completed by third party inspectors or may also
be completed by an authorized IDOT representative. In either
case the information from the form is used to create an LM6
by BMPR personnel. Once processed, MISTIC will provide a
test identification number(s).
