This form is the Application for Section 5311 Operating Assistance for Rural and Non-Metro Public Transportation providers.  This form is to be completed by Local Agencies and submitted to the Section Chief of Non-Urbanized Area Programs.  Article 3 Section 3 of the Illinois Compiled Statutes (30/ILCS 740  - Downstate Public Transportation Act), addresses Nonurbanized area public transportation and provides the requirement for the application and supplies a list of the minimum required information to be supplied by the grantees applying for Section 5311 Assistance.
