This form has been modified to revise the following:

 

Changed "Expected Accident Frequency (EAF)" to "Expected Crash Frequency (ECF)".

 

Removed formula from ECF calculation.

 

Added "B" Factors - Basic Values for Railroad Protection Devices" table.

This form has been modified based on user feedback.  Several areas were clarified: N/A was removed as a choice when not needed, questions were combined, etc.  Form name was changed (removed "On Site") to better reflect its actual use.
