This form is used by Police Departments throughout the state to request grant funds to conduct Roadside Safety Checks or Safety Belt Enforcement Zones during the Holiday Mobilizations throughout the Fiscal Calendar Year. This form is very similar to the TS 08, but there are a few things that we require the grantee to submit that we do not ask for on the TS 08.

On page 6, under Labor Day mobilization the Safety Belt Enforcement Zone fields were changed to Roadside Safety Check fields.
