Effective March 1, 2006, prior to awarding a contract or entering into any agreement, state agencies are required to inquire, using the vendor FEIN or SSN, the Comptroller's Offset System to determine if any delinquent debt has been placed by a state agency.  This inquiry shall be made for the prime vendor and each known subcontractor/subconsultant.
 
Consultants and subconsultants submitting Statements of Interest for the Professional Transportation Bulleting (PTB) are required to submit this form for each PTB.  The form will be emailed to SOIPTB@dot.il.gov and will be used so that any prime or sub will not be used if they are considered delinquent in debt to any state agency.
