This form has been modified as follows:

 

1.  Added category QC/QA to the Execution of Work table.

 

2.  On the second page under the Work Categories, removed 'Misc' from 005 and 017.

 

3.  On page 2 under Work Categories, removed 028 and removed the category under Epoxy Pavement Marking.

 

4.  On page 2 under Work Categories, added 027A, 027B, 027C and 027D.

 

5.  On the last page, added the Numerical Guidelines for QC/QA.
