This form provides the Jurisdictional Exception Certification to be signed by the appraiser and/or review appraiser and included in each appraisal, review appraisal and waiver valuation which depicts under which jurisdiction the valuations are being completed, along with the USPAP Standards 1, 2, and 3 checklists which are to be filled out and the appropriate checklists retained in the appraiser's, review appraiser's and/or waiver valuation valuator's work file.
