This form is completed by the finely divided minerals producer or cement producer and accompanies the PRE or PRO materials sample.  The sample testing is part of the requirements for a producer to become or maintain their approved supplier status for IDOT.

This form is completed by the cement producer after completion of testing of the PRO sample and identifies the sample tested.  It is required by BMPR Policy Memo "Portland or Blended Cement Acceptance Procedure for Qualified and Non-Qualified Plants".  The physical and chemical test results are entered into MISTIC and compared with IDOT test results for the same sample.  The PRO sample testing is part of quarterly testing requirement for a producer to maintain their status as an approved cement supplier for IDOT.  The form combines and replaces three previous forms: Producer's Test and Sample Identification for Process Control (PRO) Type 1 Cement Sample, Report of Physical tests on (PRO) Type 1 Cement Sample, and Report of Chemical Tests (PRO) Type 1 Cement Sample.
